Banks usually need “evidence of insurance” on a piece of property before they can close on a loan. To issue this we need the “mortgagee clause” information, which is the name, address and fax number of the bank, and a loan number if applicable.
Businesses are often asked to provide a “certificate of insurance” showing that their company has general liability, auto or workers’ compensation insurance. We need to know the name, address and contact information of the requestor, as well as any special requests. For instance, some general contractors request to be named as an “additional insured” on a subcontractor’s policy.
